Employment Forms

To hire an applicant, please fill out an Employee Action Form and send it to Human Resources. All forms on this page are fillable. Once the form is received, Human Resources will assign the AB 506 Training and send them their New Hire packet and Live Scan Form. Once all of these items are completed, Human Resources will send an approval to for the employee to start. Employees should not start working if they have not been cleared by Human Resources.

Upon completing your Live Scan, AB 506 Child Protection Training, New Hire Paperwork, and receiving direction from your hiring manager, please call Human Resources at the number listed below. We will schedule an appointment for you to come into the SCC office to complete your onboarding.

Please be sure to bring the following documents with you to your appointment:

  1. Completed New Hire Paperwork.
  2. Live Scan Receipt.
  3. Certificate(s) of Completion for your Child Abuse Mandated Reporter Training.
  4. The original documents you will be using for your I-9 verification (e.g., Passport, Driver’s License, Social Security Card, or any other document you will be using for your I-9 form). Click here to refer to the List of Acceptable Documents.

For hiring individuals who worked at SCC within the past 12 months.

Use this form (also included in the list above) for a CHANGE or SEPARATION of an existing employee. Once signed, fax or email to the HR office before the effective date.
