Risk Management

Welcome to the Southern California Conference Risk Management Department. This page is primarily for our pastors, local church risk management officers and volunteers. If you are just curious or have a specific concern, please feel free to browse and e-mail or call us if you have a question or concern.

In 2017, our primary focus will be on the protection of our children. There are and will be guidelines, forms and policies to make our churches safer and more welcoming to our children and youth. 

Additional information will also be included to assist our local churches to prepare for emergencies, prevent losses and recover from losses.

Requests for Certificate of Insurance (also known as Insurance Binders) require the above form. The Certificate Holder is the entity/venue requiring "proof of insurance." Please fill download the form first. Opening it is not downloading it. Once downloaded, open the form and fill it out. Click "File" and then "Send to" to email it to hr@sccsda.org

Instructions for Filling Out the Request for Certificate of Insurance Form

Once your form is completed, please email it back to HR@sccsda.org. This is a PDF-fillable document, and that is how we must submit it to the NAD. 

  • Organization Insured: SOUTHERN CALIFORNIA CONFERENCE – This is already typed in. Do not change it.
  • Type of Insurance: For events, always GENERAL LIABILITY. If unsure, leave blank and ask us.
  • Certificate Holder: Insert the information pertaining the entity or city, or county that is requiring proof of insurance, or the facility you are going to rent or use (or auto rental company). It could be either. On occasions, if you are having events at parks, races, etc., the city will require the COI. That would be the information you put in. To make easier:  Whoever is requiring you provide proof of insurance would be the Certificate Holder. 
  • Event location: Insert location if different from the certificate holder address.
  • Activity requiring certificate: Description the activity (what will you be doing at the facility), and be sure to include the beginning AND ending dates.
  • Additional Insured: You must ascertain from the requesting venue who the Additional Insured is, and they are to provide you with the appropriate wording to be included on the form.
  • Property/Equipment: This information is required if you are leasing equipment and need a Certificate of Insurance. Do not leave blanks. You will need to provide a copy of the lease as well.
  • Email Certificate of Insurance to: You will note that our office email addresses are already there. Please add your email address and the email address of the facility requesting same. COIs are only sent via email, so without the email address, the COI will not be delivered to the requesting parties.
  • Requested by: Please note that the SCC employee name is already listed there. DO NOT CHANGE.

You must provide all the information requested, or it will delay in the processing of your request. Most importantly, you must provide email addresses for the parties that are to receive a copy of the Certificate of Insurance. Finally, we do not guarantee a 24-hour or less turn-around time. Please plan accordingly. It may take up to two weeks to receive a completed Certificate of Insurance from the NAD Risk Management office.
